CHANGE: Background Checks are no longer performed by BCI. All Background Checks are now performed by the Salt Lake City Police Department. Background Checks are required for all Beer, Liquor, Private Club, and Sexually Oriented Business License. Complete Salt Lake City Corporation Application Verification form and submit to the Business License Office.
